Is Marriage Counseling Covered by Insurance?
Usually, no.Most health insurance plans don't cover marriage counseling because they classify it as a “relationship service,” not a medically necessary treatment. If both partners are healthy and the issue is purely relational, most insurers won't pay.
But that's not the full story. There are several ways couples get their sessions covered anyway.
The Diagnosis Route
If one partner has a diagnosable mental health condition (depression, anxiety, PTSD), a therapist can bill sessions as individual mental health treatment that involves the partner. The sessions look the same — you're both in the room — but on paper, insurance sees it as treating one person's condition.
This is legal and common. Your therapist can tell you whether it applies to your situation.
Medicare Covers It
If you have Medicare, couples counseling falls under family counseling and mental health care. Medicare Part B covers 80% of the cost when provided by a qualified behavioral health provider (psychiatrist, clinical psychologist, licensed clinical social worker). You'll pay the remaining 20% plus your deductible.
Online Platforms That Accept Insurance
| Platform | Accepts Insurance? | Out-of-Pocket Cost | Typical Copay with Insurance |
|---|---|---|---|
| Talkspace | Yes — Aetna, Cigna, BCBS, Anthem, Optum, TRICARE | $396–$436/month | $15–$30/session |
| OurRitual | No (HSA/FSA accepted) | $60–$78/week | N/A |
Talkspace — the insurance-friendly option
Talkspace is the clear winner for insured couples. They accept coverage from most major providers for couples therapy, which can drop your monthly cost from $400+ to $60–$120 in copays.
OurRitual
If your insurance won't cover it, OurRitual combines expert guidance with personalized digital tools — 300+ vetted therapists, flexible scheduling, and science-backed exercises between sessions. HSA/FSA can be used. $720 per 12 weeks (3-month plan) or $310 per 4 weeks.
Your Employer Might Cover It
Most large employers offer an Employee Assistance Program (EAP) that includes 3 to 6 free counseling sessions, including couples sessions. This benefit is heavily underused because most people don't know it exists. Call your HR department and ask.
If Nothing Covers You
- Sliding scale therapists adjust rates based on your income. Many private practices offer this, but you have to ask — it's rarely advertised.
- University training clinics charge $20–$50 per session. You'll work with a supervised graduate student, not a fully licensed therapist, but the quality is often solid.
- Open Path Collective connects couples with therapists who charge $30–$80 per session.
How to Check Your Specific Coverage
Before you book anything:
- Call the member services number on your insurance card
- Ask: “Does my plan cover outpatient couples therapy or marriage counseling?”
- If they say no, ask: “What about family therapy or mental health treatment that involves a spouse?”
- Get the answer in writing — or note the rep's name and call reference number
Different reps sometimes give different answers. If the first one says no, it's worth calling back and asking again.
